Forms 365 – SharePoint Companion connects your SharePoint lists to Forms 365, the drag-and-drop form builder that writes submissions directly into the SharePoint List or Document Library you choose.
The app adds two buttons to every list and library in your tenant:
- Open in Forms 365 — select a list item and jump straight to its bound Forms 365 form, open in edit mode for that item.
- Create Forms 365 form — start building a new form for the current list, with the site and list pre-selected in the Forms 365 builder.
Built for zero-friction approval. The app requests no permissions, reads no list data, calls no Microsoft Graph APIs, and contains no telemetry. Its only function is to open Forms 365 in a new tab with the current site and list context — all lookup and identity handling happens in Forms 365 after you land.
Who it's for: Microsoft 365 teams that capture data into SharePoint — service requests, inspections, onboarding, compliance, intake forms — and want a modern form-filling experience (validation, conditional logic, file uploads, public share links) instead of the default list form.
Requirements:
- SharePoint Online (Microsoft 365). The app is installed tenant-wide by a SharePoint administrator.
- A Forms 365 account is required to build and open forms. Sign up free at public.forms365.ai — the app itself is free, and Forms 365 offers a free plan; paid plans with additional features are available separately.
Getting started: after installation, open any SharePoint list, select an item, and choose Open in Forms 365 — or choose Create Forms 365 form to build your first form for that list.
